Before your first visit
Once your appointment is scheduled, our team will send your new patient forms directly to you by text or email. You can complete them digitally before your visit, helping us get to know you, review your needs, and make your first appointment as smooth as possible.
To complete your forms, please have your basic contact information, medical and dental history, insurance details, and any relevant health information ready. Completing everything ahead of time helps us stay prepared for insurance verification, treatment planning, and your care from the moment you arrive.

We accept medical insurance
Because we focus on sleep and TMJ-related care, Valley Sleep and Airway primarily works with medical insurance rather than dental plans. We handle the paperwork and pre-authorizations for you, making the process as smooth as possible. Our team is highly experienced in navigating insurance and will walk you through every step.
While we are not in-network with any specific medical plans at this time, we do accept a wide range of carriers and can help determine your eligibility. If you’re unsure what’s covered, just reach out. We’re happy to help clarify your benefits and advocate for your care.
